Georgia Medical Cannabis Certification Made Simple
Georgia’s Medical Cannabis Program allows patients with certain qualifying medical conditions to obtain a Georgia Medical Cannabis Patient Registry Card. This card allows patients to legally possess approved medical cannabis products containing up to 12,000 milligrams of THC, including oils, capsules, vaporization products, and more.
To obtain a registry card, you must first be evaluated by a qualified Georgia physician. Wismar Med provides these convenient online evaluations for $99. You are only charged if you meet the qualifications and your certification is submitted to the Georgia Medical Cannabis Patient Registry.
Qualifying Medical Conditions
If you have one of the qualifying conditions below, you may be eligible for a Georgia Medical Cannabis Patient Registry Card.
- Amyotrophic lateral sclerosis (ALS)
- Alzheimer's disease
- Autism spectrum disorderwhen diagnosed for a patient at least 18 years of age, or severe autism when diagnosed for a patient under 18
- Any cancer except skin cancerunless the skin cancer is metastatic
- Epidermolysis bullosa
- Patient is in a hospice programMedical care for patients who are nearing the end of life, with a focus on comfort rather than cure. Inpatient or outpatient.
- Human immunodeficiency virus (HIV)diagnosed as stage III
- Inflammatory bowel disease
- Intractable painChronic pain that has not responded adequately to standard medical treatments.
- Lupus
- Mitochondrial disease
- Multiple sclerosis
- Parkinson's disease
- Peripheral neuropathywhen symptoms are diagnosed as severeSevere nerve damage that causes burning, numbness, tingling, or pain, often related to diabetes or certain chemotherapy treatments.
- Post-traumatic stress disorderresulting from direct exposure to or witnessing of a trauma, for a patient at least 18 years of age
- Seizure disordersrelated to a diagnosis of epilepsy or trauma-related head injury

Your payment method will be authorized for $99, but you’ll only be charged if you qualify and we submit your certification to the Georgia Medical Cannabis Patient Registry.

Create your Elation Health account to complete your forms, upload your ID and medical records, and securely communicate with us. If you don’t receive your invitation within a few minutes, please check your spam or junk folder.

Complete Your Forms & Upload Records
Complete all required forms and upload medical records documenting your qualifying medical condition. To avoid delays, please upload your records before your appointment. Appointments may be rescheduled if the required records have not been received.

Attend Your Telehealth Evaluation
During your appointment, we’ll review your medical history, discuss your qualifying condition, answer your questions, and determine whether you meet Georgia’s eligibility requirements.

Receive Your Registry Card
If you qualify, we’ll submit your certification to the Georgia Medical Cannabis Patient Registry. The Georgia Department of Public Health will then email you instructions to pay the $30 state fee plus the $3.75 online processing fee. After payment is received, your Medical Cannabis Registry Card is typically mailed within approximately 10 business days.

What does a Georgia Medical Cannabis Registry Card allow?
A Georgia Medical Cannabis Registry Card allows qualified patients to possess medical cannabis containing up to 12,000 milligrams of THC, as permitted under Georgia law. The card does not authorize recreational marijuana use and applies only to products allowed under Georgia’s Medical Cannabis Program.
Your Registry Card is valid for 5 years, after which you’ll need to be re-evaluated and recertified if you continue to qualify.
The card is issued by the Georgia Department of Public Health, not Wismar Med. Georgia charges a $30 state fee plus a $3.75 online processing fee, and these fees are not covered by health insurance.
Your Registry Card is valid only under Georgia law. It does not make cannabis legal under federal law or permit you to transport cannabis across state lines.
Who can qualify?
Georgia residents with one or more qualifying medical conditions may be eligible for certification after a physician reviews their medical history, supporting medical records, and completes a telehealth evaluation.
What medical records do I need to submit?
You should provide medical records documenting your qualifying medical condition. Examples include office visit notes, specialist records, pathology or biopsy reports, imaging reports, treatment records, or hospital records. Instructions for uploading your records through your Elation Health account will be provided after scheduling.
How do I get my medical records?
You can request your records from your current or previous healthcare provider. Depending on the practice, you may be able to request them through a patient portal, medical records department, email, or fax. You may also ask your provider to send your records directly to Wismar Med using the instructions provided after scheduling.
What if I’m not sure whether my condition qualifies?
If you’re unsure whether your condition meets Georgia’s qualifying criteria, you can contact us before scheduling. You can also review the qualifying conditions listed on this page. Final certification is determined after reviewing your medical history, supporting medical records, and completing your telehealth evaluation.
Do I need a referral from my doctor?
No. A referral is not required to schedule a Georgia Medical Cannabis evaluation with Wismar Med.
Does the evaluation fee guarantee that I will receive a Medical Cannabis Registry Card?
No. The evaluation fee does not guarantee certification. Your payment method will be authorized for the $99 physician evaluation fee, but you will only be charged if you qualify and Dr. Tramaine submits your certification to the Georgia Medical Cannabis Patient Registry.
What happens if I don’t qualify?
If you do not qualify, your certification will not be submitted to the Georgia Medical Cannabis Patient Registry, and your payment method will not be charged the $99 physician evaluation fee.
Can my appointment be rescheduled?
Yes. To avoid delays, please upload your required medical records before your appointment. Appointments may be rescheduled if the necessary records have not been received in advance.
Is my information secure?
Yes. All forms, medical records, and communications are securely managed through Elation Health, our HIPAA-compliant patient portal.
How long does the process take?
Most telehealth evaluations take approximately 15–20 minutes. If you qualify, we’ll submit your certification to the Georgia Medical Cannabis Patient Registry. The Georgia Department of Public Health will then email you instructions to pay the $30 state fee plus the $3.75 online processing fee.
